Improving health of older Australians
West Moreton Aged Collaborative
In partnership with West Moreton Health, the PHN hosted the inaugural West Moreton Older Person’s Care Collaborative meeting in February 2021.
Over 30 organisations attended with discussions focusing on how stakeholders can best work together to support sustainable, high quality and integrated health services for older people in the West Moreton region.
The workshop provided a platform for people to share what is currently working well, current challenges and the future risks.
Palliative Care in RACFs
The PHN continues to support in-hours and after-hours palliative care services for residents of residential aged care facilities (RACFs) in the Ipswich and Lockyer Valley region.
The service, provided by CiMAS Home Nursing Services, provides residents access to coordinated and high-quality palliative care that is timely, patient-centred, and based on clinical need and the patient’s GP palliative care plan.
AGES Model of Care Project
The PHN continued to support the Darling Downs Health’s Acute Geriatric Evaluation Service (AGES) Model of Care Project in the after-hours period in 2020-21.
The Project provides a ‘Hospital in the Nursing Home’ model of care, focused on providing a range of services such as telephone triage, telehealth consultations, rapid assessments in the RACF by the AGES team, and education and training support for RACF staff.
Care at the End of Life Collaborative
The West Moreton Care Connect website was launched in 2020 and provides a complete directory of local services, support networks and organisations that can assist people with a life-limiting illness, as well as their carers and families.
The website is the only one of its kind in the region and is an initiative of the West Moreton Region Care at the End of Life Collaborative, which was set up in 2018 to improve end-of-life care by addressing gaps in care and access as well as the capability of health workers and professional caregivers.

West Moreton Aged Care Symposium
Over 80 health professionals and care givers attended a West Moreton Aged Care Symposium in Ipswich to provide residential aged care facility (RACF) staff and primary health care nurses with best practice training.
The Symposium featured hands-on skills sessions, expert guest speakers and a trade exhibition for attendees to learn more about topics including palliative care management, dementia, wound care, syringe drivers, stoma care, catheter and PEG insertion, and more.

Empowering Elders to improve their health
A program funded by the PHN empowered Elders living in the Ipswich community to take charge of their health and wellbeing.
Steve Royle, an exercise physiologist for Health Management Solutions Queensland, facilitated a special eight-week Healthy Eating and Activity Lifestyle Program (HEAL™) with Elders to improve their health and wellbeing, and manage their chronic conditions.
